歯牙萌出に興味ある口腔内所見を認めたクレチン症の1例

Abstract

The authors report a case found as cretinism at the age of 34 years, especially on the oral and maxillofacial aspects. The patient, a female, was referred to our clinic at the age of 35 years with the complaints of prolonged retention of the deciduous teeth and multiple dental caries. When the patient suffered from pseudo intestinal obstruction at the age of 34 years, she was found to suffer from cretinism in the hospital, from where she was referred to us. The patient had been untreated until then. The patients height was 114 cm and body weight 26 kg. The face was edematous and the nose was flat, which were characteristic of cretinism. The bone age was matched as ten years. IQ was below sixty, which meant that mental age was estimated to be about two or three years. Oral findings were as follows : 1) residual teeth : ＜EDC＞＿＿＿-｜＜CDE＞＿＿＿-, ＜EDC＞＾＾＾-｜＜CDE＞＾＾＾- 2) impacted teeth : ＜543＞＿＿＿-｜＜345＞＿＿＿-, ＜8543＞＾＾＾＾-｜＜45＞＾＾- 3) multiple dental caries 4) Radiographic examination showed that the border of cortical bone and cavernosus bone was not clear, and that the maxillary and mandibular bone was osteosclerotic. As the serum levels of T＿3, T＿4 and TSH were normalized, extraction of caries teeth was routinely performed. Wound healing was good.
