胃癌と十二指腸癌が重複した1症例

Abstract

A case of double cancer of the stomach and the duodenum is reported. A 72-year-old man developed vomiting in August 1986 and epigastralgia in October, and he was referred to our hospital for further examination and treatment. An upper gastrointestinal X-ray series revealed ulcerative tumor in the anterior wall of the gastric antrum and in the infra-ampullary portion of the duodenum. The pathological diagnosis of the biopsy specimen was adenocarcinoma of the stomach and benign duodenal tumor. However, the possibility of duodenal cancer had been suspected from the macroscopic findings. He died of pneumonia and renal failure in December 1986. The macrospecimen at autopsy showed 4.7×4.4 cm Borrmann 2 cancer in the stomach and 2.0×2.3 cm Borrmann 2 cancer in the duodenum. Histologically they were well differentiated tubular adenocarcinoma and moderately differentiated tubular adenocarcinoma respectively. But there were no invasions to the pancreas and common bile duct or metastasis to the lymph nodes, liver and so on. This type of double cancer is very rare, and to our knowledge, only nine cases including our case have been reported in Japan.
