高齢者の運動器疾患における慢性疼痛に関する文献的考察

Abstract

【目的】高齢者の運動器疾患における疼痛アセスメント方法の検討を行い、運動器疾患における疼痛出現率の実態と、看護介入効果に関する文献的考察を行うことを目的とした。【方法】PubMed、EBSCO、医学中央雑誌をデータベースとし、文献検討を行った。【結果】高齢者の疼痛アセスメントスケールは、視覚アナログスケールと数値的スケール及びマクギル疼痛質問票とSF-36が多く用いられていた。慢性疼痛の出現率では、腰痛や背部痛が一番多く、次いで下肢痛が認められ、57.9%が症状は改善しなかった。看護介入効果の検討では、教育介入と身体機能回復への介入、罨法や音楽聴取による介入により疼痛緩和が認められた。【結論】さらなる高齢化の進展に伴い、標準化された疼痛スケールによる慢性疼痛の実態調査、看護師を対象とした疼痛評価や緩和方法についての教育プログラムの開発と検証が望まれる。

【Purpose】The study aimed to examine pain assessment scales for musculoskeletal diseases in the elderly, the prevalence of pain in the same, and nursing intervention effects through a rature review. 【Methods】 We searched the following electronic databases for relevant literatures: MEDLINE, EBSCO, and Japan Medical Abstracts Society. 【Results】 The asssessment scales commonly used for the elderly were the visual analogue scale, numeric rating scale, McGill Pain Questionnaire, and Short Form 36. The highest prevalence rates of chronic pain were observed in the lower and upper back, followed by the legs. However, 57.9% of the patients had no improvement in their conditions. The effects of nursing intervention, educational intervention, recovery of physical function, hot compress, and listening to music were assessed and found to relieve pain. 【Conclusions】A super-aging society is impending; therefore, surveys on chronic pain performed by using standard scales are important. Moreover, educational programs for nurses are needed to promote pain assessment and relief.
