母乳栄養確立を阻害する要因の検討と母乳育児における自己管理方法の実態

Abstract

本研究の目的は、母乳育児において母乳栄養確立を阻害する要因を明らかにするとともに、母乳育児における自己管理方法の実態を知り、母乳栄養確立に向けて産後の母親に適した自己管理に向けた支援方法を検討することである。A保健センターの4か月健診を受診する母親を対象に質問紙法調査を行った。配布数130に対し回収数は119であり、97名を分析対象とした。「年齢」「初経別」「前回の児の栄養法」「分娩経過」「妊娠中の母乳栄養の希望」「退院時栄養法」「乳頭の形(正常・異常)」 の7つの項目が母乳栄養確立を阻害する要因であった。また、自己管理知識率を比較すると、現在の栄養法が「母乳のみ」と「混合・人工乳」ともに9項目が特に知識率が低かった。今回の結果を基にこれらの要因について今後支援を強化していくとともに自己管理方法を指導し、退院後に実施できるととを確認していく必要がある。

【Purpose】The purpose of this study is elucidate the factors that prevent the establishment of breastfeeding, to examine self-management methods in breastfeeding, and to nvestigate the methods of care for establishing breastfeeding nutrition. 【Methods】A questionnaire survey was conducted on mothers visiting for their 4-month routine checkup. 【Results】The survey identified seven factors that were significantly diffrent in establishing breastfeeding nutrition: "age," "menarche," "whether the mother practiced breastfeeding for the older child," "delivery method," "will to breastfeed," "feeding method at the time of discharge," and "nipple shape." Furthermore, in the comparison of current feeding methods and self-management methods, low factors were nine. 【Conclusions】A significant difference was found in the self-management scores between primipara and multipara women, with multipara women having lower self-management scores. Hence, it essential to instruct both multipara and primipara women of the same self-management methods during hospitalization.
