インスリン非依存性糖尿病患者での高血圧合併について : 自験例での検討と文献的考察

Abstract

To characterize the clinical features of hypertension in patients with non- insulin-dependent diabetes melitus (NIDDM), we studied 645 hospitalized NIDDM patients consecutively. For each patient, we investigated the presence and etiology of hypertension, age on admission, duration of diabetes, presence of obesity (body mass index ; BMI), and family history of diabetes and/or hypertension. The overall incidence of hypertension in NIDDM patients was 54.2%, which is compatible with some previous reports. When classified according to the etiology of hypertension, essential hypertension accounted for 82.7% of the hypertension cases among NIDDM patients, while renal hypertension re- presented 12.5%. The incidence of hypertension in patients with NIDDM showed a correlation with age on admission, BMI, and family history of hypertension, but no relation- ship to the duration of diabetes. We discuss the pathogenesis of hypertension in patients with NIDDM based on relevant published studies.
